Quad-Mix (Papaverine / Phentolamine / Alprostadil / Atropine)
30 mg / 2 mg / 20 mcg / 200 mcg per mL – 5 mL Vial is a compounded injectable formulation used in the evaluation and treatment of erectile dysfunction (ED), particularly in diagnostic intracavernosal injection (ICI) testing or therapy-resistant cases. Each component contributes a distinct pharmacologic effect on penile smooth muscle and vascular tone.
Papaverine (30 mg/mL) – a non-specific phosphodiesterase inhibitor that relaxes vascular and corporal smooth muscle, increasing blood flow.
Phentolamine (2 mg/mL) – a non-selective alpha-adrenergic antagonist that reduces sympathetic tone, promoting vasodilation in penile tissue.
Alprostadil (20 mcg/mL) – a prostaglandin E1 analog that stimulates cyclic AMP production, leading to smooth muscle relaxation and increased penile blood flow.
Atropine (200 mcg/mL) – an anticholinergic agent included in some compounded protocols to minimize reflexive detumescence and enhance erection duration.
Quad-Mix is commonly used for diagnostic testing of erectile function, to differentiate between psychogenic and organic ED, or therapeutically in men unresponsive to oral agents. Administration is via intracavernosal injection under medical supervision, with careful monitoring for priapism, hypotension, or local tissue reactions.
